Ethical concerns with ketamine treatment for depression

A Google "ketamine depression" search will bring up the two kinds of narratives about ketamine. The first kind reads like an advertisement. The patient was suffering, nothing worked, and then ketamine saved their life from treatment-resistant and major depressive disorder in a single afternoon. These narratives are widely broadcast from reputable medical institutions like Texas Medical Center, calling it a "miracle drug" (TMC News, 2019), to emerging trends on TikTok, where influencers describe ketamine as a "brain reboot" (Kloud Health, 2025), popularizing microdosing of ketamine as an intervention for treatment-resistant and major depressive disorder. The second kind of narrative seen in regulatory bodies in the past decades frames ketamine as a hazard, a pure club drug with no utility for practical clinical application. Ketamine has resulted in the death of celebrities in the past, most notably Matthew Perry, drawing an influx of attention to ketamine. Ketamine has potential implications for addiction, and the clinics popping up to sell ketamine are basically predatory. The framing of ketamine's addictive nature shows up in NPR reporting, in CNN coverage after Perry's death, and in the regulatory language around Schedule III substances (CNN, 2024; NPR, 2024).

Both narratives are products of news coverage patterns that systematically eliminate the clinically significant aspect of the treatment; the plasticity window, the most significant thing ketamine does is not the immediate relief, which wears off within hours. It is what happens after. Ketamine triggers a temporary window of neuroplasticity, a period of approximately 24 to 72 hours during which the brain becomes flexible enough to form new synaptic connections (Savalia et al., 2021). If a patient engages in therapy or structured behavioral change during that window, there is evidence that the effects can be long-lasting. If they do not, the window closes, and the brain returns to its prior rigid state, leaving the patient experiencing an expensive, temporary mood improvement that fails to sustain for a few hours.

Despite the significance of the plasticity of the window, it remains largely underreported or absent from mainstream reporting. The result of these ethical failures is a reflection of the miracle narratives of ketamine, as advertised by journalists, clinics that skip the detailed explanation of the window of plasticity, and a medical culture that still treats ketamine like a magic pill rather than what it is: a tool that only works if used correctly.

The Biology of Hopelessness

A dissection of depression at the neurobiological level is fundamental to comprehending the significance of the plasticity window; this is also where most media coverage falls apart.

Depression is not just the affective feeling of sadness; it is not simply just a mood; it is a dysfunction of the neural circuitry. In a depressed brain, the reward system fails to function correctly, and this failure is localized to a small structure called the lateral habenula (Yang et al., 2018). The burst of the lateral habenula in depressed patients releases glutamate, which excites the rostromedial tegmental nucleus (RMTg), a relay in the reward circuitry. The RMTg then sends inhibitory signals to the ventral tegmental area (VTA), suppressing dopamine. Less dopamine means less motivation, less pleasure, and less ability to feel like anything is worth doing. The end point of this chain is the nucleus accumbens, the part of the brain that processes reward. In a depressed person, it is being actively suppressed by the lateral habenula's constant false alarms (Yang et al., 2018). The clinical name for this is anhedonia. The lived experience of it is waking up and not caring about anything.

SSRIs, the standard first-line antidepressant, try to fix this by increasing serotonin levels. However, serotonin is upstream and indirect. It is like trying to fix a broken brake pedal by adding more fuel. It takes weeks to show any effect, and according to the NIMH, roughly a third of patients never respond at all (NIMH, 2024).

The Circuit Breaker and the Window

Ketamine takes an entirely different approach. It is an NMDA receptor antagonist, meaning it blocks the receptors that facilitate the lateral habenula's burst firing. The inhibitory cascade downstream stops. Dopamine is no longer being suppressed. The experience of administering ketamine is often described as a sudden lifting, as if a weight were coming off their chest (Yang et al., 2018). The euphoric feeling is also often the favorite part of the story that the media widely broadcasts, but it is technically accurate only in part.

After the acute effects of ketamine wear off, the drug triggers a release of brain-derived neurotrophic factor, or BDNF, a protein that acts like fertilizer for neurons (Kang et al., 2022). BDNF promotes the growth of dendritic spines, the tiny protrusions on nerve cells where new synaptic connections form. Two-photon imaging studies have shown that after a single dose of ketamine, new dendritic spines start appearing in the medial prefrontal cortex within about 6 to 12 hours, and the density of these spines peaks around 24 to 48 hours after administration (Savalia et al., 2021; Wu et al., 2021).

To put it simply: the drug causes the brain to sprout new wiring. The rigid, stuck circuits of depression become temporarily flexible. For a brief period, roughly three days, the brain is in a state where old patterns can be disrupted, and new ones can take hold. This is the plasticity window. Moreover, by about 72 hours, it begins to close (Savalia et al., 2021). The new spines, which were not reinforced by experience or therapy, begin to retract, marking the end of the window.

So here is the question nobody seems to be asking: what is the patient doing during those 72 hours? Are they in therapy? Are they practicing new thought patterns, new behaviors, new ways of engaging with the world? Or are they driving home from the clinic and going back to the same environment and routines that reinforced their depression in the first place? Contrary to mainstream practices, particularly in pop-up ketamine clinics, where the window of plasticity is often entirely disregarded, the research strongly suggests that proper support during this window is fundamental to maximizing and solidifying the effect of ketamine. Gagnon (2025), writing in Frontiers in Psychiatry, called the widespread failure to educate patients about the plasticity window a "collective lie" in ketamine therapy.

The Myth of the Miracle

The bipolar coverage of ketamine, miracle or menace, is not just incomplete; it inflicts harmful expectations among vulnerable patients suffering from treatment-resistant and major depressive disorder. When the Texas Medical Center runs a headline calling ketamine a "miracle drug" and profiles a patient whose depression lifted after a few infusions, the reader walks away thinking the drug is the treatment (TMC News, 2019). The failure to disclose therapy, plasticity window, and the mention of what happens at the 73-hour mark when the window closes, leaves already vulnerable patients at risk of the side effects of ketamine.

Social media further perpetuates the miracle of ketamine. The language people use on TikTok and Instagram to describe ketamine, "brain reboot," "shortcut to clarity," "mental reset," all of it implies passivity (Kloud Health, 2025). The metaphors are not just sloppy; they are wrong in a way with clinical consequences, because they tell people they do not need to do anything after the infusion and that the drug will handle it.

Then Matthew Perry died in October 2023, and the coverage swung hard in the other direction. Suddenly, ketamine was a killer, a Schedule III substance, a tool for profiteering doctors and underground dealers (CNN, 2024; NPR, 2024). The narratives focused on the criminal network that supplied Perry, on the text messages between his doctors joking about how much money they could extract from him, and on the fact that his assistant had been injecting him without medical training. All of that reporting was significant. However, almost none of the reports focused on neuroscience, to question whether Perry had been told about the plasticity window, or whether any of the people treating him had a plan for what should happen during those 72 hours. The conversation stayed at the surface: access, legality, and scandal.

Researchers have quantified this pattern. Zhang and Ho (2023) conducted a systematic review of print media representations of Ketamine across the top newspapers in the U.S., the United Kingdom, Canada, and Australia between 2015 and 2020. They discovered that 68.9% of articles framed ketamine treatment in an extremely positive light, and that frequent reliance on optimistic quotes from clinicians, while giving minimal attention to long-term safety data or the limitations of existing research. An earlier study done by Zhang and Ho (2017) tracked the 15-year shift in North American print coverage, documenting how ketamine went from being a party drug to now being described as more effective than antidepressants. However, neither study has included significant coverage of the plasticity window or the role of therapy that minimizes the implication and sustains the positive effect of Ketamine.

Wilkinson et al. (2024), writing in JAMA Psychiatry, put it bluntly. Ketamine has been "hailed as a miracle treatment" even as the clinics offering it operate with almost no federal oversight and enormous variation in how they deliver care, leaving the gap between hype and regulation to compromise patient care.

What Responsible Reporting Would Look Like

New media need not start from scratch to develop measures for responsible reporting of ketamine; there are guidelines that define responsible reporting. The Association of Health Care Journalists has a model for ketamine reporting stating that reporters should always seek independent expert commentary and avoid phrases like “miracle” or “cure” unless clinically validated (AHCJ, 2020).

Breaking news venues like CNN and NPR that have covered Perry’s death extensively could have included a brief expert segment on the plasticity window; this brief expert segment could have fundamentally altered how patients approach ketamine.

Consumers should also seek source medical information from established health journalism venues such as STAT News, Kaiser Health News (KFF Health News), The Atlantic’s health section, and specialized podcast formats like NPR’s Shots blog. These established venues are specialized for reporting medical-grade information and have incentives to verify the validity and reliability of their reporting to uphold their reputation as leaders in the field and to prevent potential legal concerns arising from misinformation.

The Wild West of Clinics

Officially, the informed consent comprises treatment criteria, potential health implications, and how they will participate. For the administration of an exogenous pharmacological chemical like ketamine, the process should include three things: first, ketamine creates a temporary window of brain flexibility; second, this window of plasticity lasts roughly 24 to 72 hours; and third, engaging in therapy or structured behavioral work during this window may be what makes the difference between lasting improvement and a temporary high. If clinics are failing to disclose these three criteria to patients, patients are consenting to a treatment they do not actually understand, which is a direct violation of the APA's ethical principles.

Many clinics choose to minimize the information disclosure of ketamine because the commercial ketamine industry runs essentially on cash payments and off-label prescriptions.

Unlike esketamine (Spravato), which the FDA approved in 2019 and which requires administration in certified settings with post-dose monitoring under the REMS program, off-label IV ketamine has no such requirements (Wilkinson et al., 2024). A clinic can open, administer ketamine, and send the patient home without ever mentioning BDNF, dendritic spines, or the plasticity window. Some clinics do not even have a psychiatrist on staff, and some do not offer therapy at all. In those settings, the most essential part of the treatment, the part that actually drives long-term change, is being systematically ignored. Gagnon (2025) called this a failure to "realign clinical practice with neurobiology," reflecting the existing mismatch between science and the industry.

Reframing Ketamine as a Catalyst

Words matter in medicine. When a patient hears "miracle," they expect to be saved. When they hear "catalyst," they expect to participate. That difference in framing alters how people perceive their behavior during the most critical hours of their treatment. A patient who understands that ketamine opens a window, not closes a case, is more likely to show up for therapy the next day, more likely to pay attention to their environment, and more likely to try new patterns. At the same time, their brains are still flexible enough to encode them.

Journalists have a role here. When reporting on ketamine, they should be asking clinics: Do they offer therapy integration? Do they tell patients about the plasticity timeline? What does the follow-up look like in the 72 hours after an infusion? Clinicians have a role too. Informed consent paperwork should include the biological mechanism of the drug, on top of the list of side effects. It should be made clear that ketamine is not the end of depressive treatment, but the beginning. Patients deserve to be informed about the neurobiological profile of ketamine to truly comprehend the exact mechanism of how ketamine might assist in alleviating their depressive symptoms.

Conclusion

It has been well-documented that ketamine can relieve depressive symptoms rapidly in patients who have not responded to SSRIs, SNRIs, or psychotherapy. However, the plasticity window, the 24-to-72-hour period of heightened neural flexibility after administration, is the most clinically significant and most underreported part of the treatment. According to Gagnon (2025), fewer than half of surveyed ketamine providers include information on plasticity in their patient education materials. That number needs to change to match the clinical reality and administration of ketamine accurately and to minimize potential hazardous implications of ketamine, like addiction and abuse. Journalists should integrate reporting on the plasticity timeline alongside access narratives. Off-label ketamine could also be further regulated by FDA-issued guidance on informed consent standards like the Spravato REMS program to maximize the security of ketamine administration. Every infusion should come with a structured plan for the 72 hours that follow, because the drug is a catalyst that purely opens the gateway for positive outcomes; it does not directly result in better outcomes.

All essays
